Axis Bank Fixed Deposit Form
The Axis Bank FD Form is available both online and offline.
For Existing Customers
The existing customers of Axis Bank have to submit FD account opening form and fill in the below mentioned details. Existing customers of Axis Bank who are Resident Indians, HUF (Hindu Undivided Family) and Overseas Indians can apply for Axis Bank FD.
Type of account:
- Under this customers would have to select the type of account they want to open. Customers would have to tick one of these: Fixed deposit/ Recurring Deposit/ Tax saver FD/ FD Plus (cannot be closed before maturity date)/ NRE Deposit/ FCNR Deposit/ RFC Deposit
Customer Onboarding Section:
- Under the primary applicant section, customers have to fill their Name, Customer ID and PAN. If PAN is not available, customers can fill Form 60 or 61 separately. Customers should also note that PAN/Form 60/61 is not required for NRE/FCNR Deposits.
- Under the joint applicant section, customers have to fill their Name and Customer ID.
FD Account Opening Section:
- Customers have to select the form of Fixed Deposit they would need: Receipt/ Physical advice/E-advice
- Next, the customers would have to select the mode of operation they would want to have: Self/ Jointly by all/ Former/Survivor/ minor account operated by guardian/ Others
Type of Deposit Account:
Under this, the customers would have to fill in the following details:
- Deposit/Installment amount
- Period of FD in months and days
- Interest payout: Cumulative/ Monthly/Quarterly/ Half-yearly/Simple
- Auto renewal: Yes/No
- Auto closure: Yes/No
- Number of times customers want to auto renew their FD
- If customers want their TDS to be deducted, write ‘Y’
- If TDS is to be deducted from Savings Bank/Cash Access Account/Current account, mention the account number
- Mark tick, if TDS is to be deducted from FD account
- Write N, if TDS is not to be deducted and mention TDS exempt reference number and TDS exempt submission date
- Form 15H/G: Yes/No
Interest Payments/Maturity Proceeds:
Under this, customers have to mention where they would want their interest payments and maturity proceeds of the FD to get credited.
- If Axis Bank Savings Account, mention account number
- If customers want the credits in the form of Demand Draft, tick on ‘Issue DD’
- Next, customers would have to sign the form
Remittance Details (applicable only for the Overseas Indians):
This section should only be filled by the existing overseas Indian customers. Under this, the customers have to mention the mode of payment and currency type.
- Payment by: Demand Draft/ Cheque/ Traveler’s Cheque/ Foreign Currency/ Debit to Account (mention account number)
- Amount in Rupees
- Currency type: Indian Rupee/ Foreign Currency (specify foreign currency)

Initial Payment Details:
Under this, customers have to fill in the payment details of the Fixed Deposit Account they are opening.
- Deposit amount in numerals and words
- Mode of payment: Cash/Debit from existing account (mention account number)
- If customers are opening FD account with cheque, they have to mention cheque number, date, bank name, branch and signature
Nomination:
Under this, customers have to mention the nomination details. Customers should note that only one nominee is permitted. Customers would also have to sign the nomination form even in the case of no nomination. The customers would be required to fill in the below mentioned details:
- Name of the depositor
- Address of the depositor
- Nominee’s name
- Nominee’s address
- Relationship of the nominee with the depositor
- Age of the nominee
- Date of birth of the nominee, if he/she is a minor
- After this, the customer (depositor) and witness would have to sign the FD form along with mentioning their name, address, and date and place of signature.

Customers who do not have any existing account with the Axis Bank can also open a fixed deposit account with the bank. - If the customer is salaried, they have to tick in front of the type of company they work for: Pvt Ltd/ Public Ltd/ Proprietorship/ Partnership firm/ Public sector/ Government/ Multinational/ Others (specify others)
- Next, customers have to mention their annual income in numerals
- If the occupation of customers is self-employed, they have to mention the nature of business: IT/ Professional service provider/ Agriculture/ Bullion or Gold jewelry/ Stock broker/ Real estate/ Trader/ Money lender.
- Next, self-employed customers have to state the number of years of running their business
- After that, customers have to specify their source of funds: Salaried/ Business income/ Agriculture/ Investment income/ Others (specify others)
Know Your Customer:
Under this section, customers have to mention whether they are opening their FD account through e-KYC or not. And then, mention the transaction ID.
If customers are not opening an FD account through e-KYC, then they have to attach the photocopies of the KYC details with the form and produce the original documents for verification at the bank.
Type of Deposit Account:
In this section, customers have to mention the type of fixed deposit account they want to open with the bank. The customers have to fill in the following details:
- Deposit amount in digits
- Period/tenure of deposit in months and days
- Amount of deposit in words
- How customers would want to have their interest payout: Cumulative/ Monthly/ Quarterly
