The National Pension Schemes (NPS) is a pension plan that any Indian citizen can opt for to secure their financial future. Under existing NPS rules, subscription to this nation-wide pension scheme is mandatory for government employees but for other individuals, it is an optional subscription. Non Resident Indians too can opt to subscribe for this pension program subject to key terms and conditions. As with any other scheme, there are a few different documents that need to be filled out in one wants to sign up for the scheme or update details such as nomination on existing records. In the following sections, some of the key National Pension System Forms and their key features will be discussed.
There are three key types of NPS forms that are important for prospective and existing subscribers of the National Pension System – NPS registration forms, NPS request forms and NPS details of change forms. In some cases, National Pension System forms can also vary based on whether the subscriber has signed up for the NPS all citizens model or the NRI NPS scheme. The following are key details about some of the most important National Pension System (NPS) Forms:
NPS CSRF Form is the common subscriber registration form (CSRF) which needs to be filled out and submitted by those who are interested in signing up for the National Pension System. Being a common subscriber form, any NPS applicant irrespective of whether they opt for the Central Government NPS, State Government NPS, All Citizen Model, Corporate Sector NPS or NPS Lite (GDS) are required to fill out this form. The online NPS CSRF Form is free to download and subsequently you have to print, fill it out and submit it to start the NPS registration process.

NPS allows subscribers to invest in two separate accounts – Tier 1 and Tier 2. The Tier 1 account is compulsory for NPS subscribers while Tier 2 Account is optional. In case a NPS subscriber seeks to activate the Tier 2 account, then the NPS Annexure 1 to CSRF 1 Form will have to be filled out and submitted.

In order to activate the Tier II account, new NPS subscribers have to provide some key information such as the following:
Once filled out, applicants can sign the form and submit it with the applicable CRA (Central Recordkeeping Agency) or CRA-FC (Central Recordkeeping Agency – Facilitation Centre) to activate the Tier II account under NPS.
The NPS Annexure II to CSRF 1 Form is to be used by new subscribers of the National Pension System in case they decide to provide additional details apart from those mentioned on NPS CSRF 1. As the name suggests these additional details need to be provided as they are not available in the common NPS registration form – NPS CSRF1.

This NPS form includes fields for entering key details -Father’s Name, Mother’s Name and request for PRAN (Permanent Retirement Account Number) card to be printed in Hindi. None of these sections is mandatory and they need to be filled out only in the following cases:
The NPS form titled Annexure III to CSRF 1 can be used by NPS applicants to provide nomination details of the NPS account. This is to be filled out, signed and submitted along with the CSRF 1 Form used for NPS registration. The form allows applicants to provide key details (Name, Address, Relationship and Percentage share of NPS account balance, etc.) of up to 3 nominees. Acceptable nominees under NPS Rules include close family members such as father, mother, spouse, siblings and children.

The NPS Annexure S5 form is used as a covering letter which is attached by the DDO (Drawing and Disbursing Office) or NL-CC (NPS Lite Collection Centre) to new PRAN (Permanent Retirement Account Number) registration forms before sending them to the NPS nodal office. These forms are for office use only and new NPS subscribers do not have to fill out this form.
The NPS Annexure S6 Form is filled out by authorised signatories at PAO (Pay and Accounts Office) under the Central/State Government/Union Territories. The key responsibility of a PAO is to maintain records of contributions made by NPS subscribers. The NPS Form S6 is filled by and signed/stamped by an authorised signatory at the PAO and contains details of the number of new PRAN registration forms received from different DDO along with Registration number of each DDO from whom the forms have been obtained. The Annexure S10 NPS form has to be filled out and submitted in case a NPS subscriber wants to activate his/her NPS Tier II account. The Annexure S10 NPS Form is different from the Annexure I to CSRF 1 NPS Form as the S10 form can only be submitted by existing subscribers who have a NPS Tier 1 account in good standing. For NPS applicants who want to activate their NPS Tier II account, the correct form is Annexure I to CSRF 1.

The above details are similar to the ones required when a new NPS applicant fills out and submits the Annexure I to CSRF 1.
The National Pension System allows non-resident Indians (NRIs) to subscribe to this market-linked pension scheme. In order to subscribe to NPS, NRI applicants need to fill out the NPS NSRF Form. One should note that NRI subscribers to this pension scheme need to have an operational NRO or NRE account in order to contribute to their NPS Tier I or NPS Tier 2 bank account. Additionally, NRI NPS applicants also need to have valid PAN and Aadhaar in order to sign up for this pension plan.

NSRF Annexure I NPS Form is similar to the Annexure I CSRF 1 and allows NPS subscribers to provide additional details which could not be provided in the standard NSRF Application Form. The details that may be filled out in Annexure I of the NSRF include:

The Annexure II to NSRF allows NRI applicants of the National Pension System to provide details of multiple nominees (up to 3) and the percentage share of each nominee for the NPS account balance. This additional form is required as the NSRF form contains the space to add just one nominee. Thus NRI NPS applicants can use the Annexure II to NSRF NPS Form to provide details of multiple nominees.
