Identity proof: A valid photo identity proof is mandatory for a PAN card application. The ID proof should bear the name of the applicant, photo, date of birth, signature and father’s name of the applicant. Any change or correction required in PAN should be supported by a relevant document. The applicant may submit either any one or the combination of relevant documents as mentioned below:
Age Proof: A valid date of birth proof is mandatory to be provided along with the PAN card application. The valid age proof can be any one of the below-mentioned documents.
Address proof: Any one of the below-mentioned documents is required to be furnished as a proof of correspondence address.

Ans. No, in case of lost PAN Card number, you can apply for the reprint of the PAN Card.

Ans. Yes, you can download an e-PAN card from the official website of NSDL by applying for the category of existing PAN cardholders. Moreover, you are required to fill and submit the NDSL PAN Card lost form and have to pay the processing fee as well.
