The Claim procedure under this particular Insurance plan is as follows:
- For Cashless Basis
- The first step is to get admitted into a hospital. Check which hospitals are network and which are non-network on maxbupa.com.
- Use your Max Bupa Health card or share details such as policy number, passport details, and voter id details.
- Identification needs to be done by submitting the pre-authorisation request form. It will be validated at the network hospital.
- The verification process will be conveyed by the network hospital via fax or e-mail. Text message and E-mail will also be conveyed within a span of 30 minutes.
- As per policy terms, the claims will be settled with the adequate formalities.
2. For Remuneration basis
- In the case of hospitalization, inform the Insurance Company within a period of 48 hours of admission in the non-network hospitals.
- After getting admitted, the payment needs to be made directly to the hospital themselves.
- On discharge collect all the documents, invoices and medical reports which are needed in the claim filing.
- Send the respective documents along with the filled claim form and a valid id proof.
- The insurance company will review your respective documents for the claim procedure.
- As per the policy terms, the claim will be settled along with the formalities.

- Claim form which is duly filled and signed.
- All the respective KYC documents.
- The original hospital discharge summary [final] or all the self-attested copy [if the originals are already submitted with the insurer respectively].

- All the consultation notes issued
- Investigation reports from outside the hospital before hospitalization.
- The first consultation note along with the medical records since the onset of complaint.

- The copy of the medico-legal certificate duly attested to the concerned hospital.
Exclusions under Max Bupa Criticare
- AIDS and HIV are excluded under this policy.
- All attempts of suicide or any intentional self-injuries by the policyholder are excluded.
- In a scenario of failure to seek or follow the medical advice.
- Breach of law
- Any activities which the insured participates under criminal intent are also excluded under this policy.
- Nuclear contamination is also excluded under this policy.
- All involvement in hazardous activities or sports is excluded under this policy.
- Participation in aviation/marine activities other than as a crew and fare passenger is excluded under this policy.
- Involvement in Narcotics unless medically prescribed
- All acts of the foreign enemy are excluded under this policy.
- Involvement in civil wars, riots or strikes is excluded under this policy.
How long does the claim procedure take place?
The claim procedure will take the time as per the terms and conditions in the policy clause along with the formalities associated with it.

- This policy covers 20 critical care illnesses such as Cancer of specified severity, Loss of speech, Deafness, Open Chest CABG, Kidney failure which may require regular dialysis, End Stage Lung as well as Liver diseases, Major burns, First heart attack with respect to specified severity, Open Heart replacement/Repairing the heart values, Stroke associated with symptoms of permanent nature, Limb permanent paralysis, Transplant associated with bone marrow or major organ.
